Access Lifecare provides case management as a part of our supported independent living program. We will prepare an individual care and support plan in coordination with your treating health team, to set out your goals and strategies to best support you. We will provide you with daily support to manage your mental health and emotional wellbeing and will tailor the plan based on your individual needs. We will maintain your privacy and confidentiality as per the professional standards.
